Why the surgeon's certification matters more than the clinic's brochure

Training, not a weekend course

A plastic surgeon completes general surgery before six further years of supervised plastic surgical training. Cosmetic technique is taught inside that reconstructive grounding — which is what makes the difference when an operation does not go to plan.

The whole picture, not the one feature

Cosmetic surgery is the art and plastic surgery the science of building self-confidence. Consultation begins with self-image and function, not with a menu of operations; sometimes the right recommendation is no surgery at all.

How do I know a surgeon is actually a plastic surgeon?

Ask for the MCh or DNB in plastic surgery and the state medical council registration number. A dermatology, dental or general surgical qualification is not the same training, whatever the clinic is called.

Will there be a visible scar?

Every cut leaves a scar. The work is in placing it in a natural crease or a line clothing covers, closing it without tension, and managing it for the twelve months it takes to fade. Where a scar cannot be hidden, that is said before surgery, not after.

How long before I can go back to work?

Between five days and four weeks depending on the operation and on what your work asks of your body. Each procedure page carries its own week-by-week recovery timeline.

A reconstructive surgeon who does cosmetic work

Dr. Adhish Basu is a board-certified plastic surgeon with a special interest in cosmetic surgery, and more than ten years of practice in plastic and cosmetic surgery.

After completing his MBBS at Medical College, Kolkata, Dr. Basu took his MS in General Surgery at the Jawaharlal Institute of Postgraduate Medical Education and Research (JIPMER), Pondicherry, and then cleared the MRCS (UK) intercollegiate examination.

He trained in Plastic and Reconstructive Surgery at the Postgraduate Institute of Medical Education and Research (PGIMER), Chandigarh, where he became expert at treating complex injuries of the face and limbs, congenital deformities, burn injuries and reconstruction after the removal of tumours. He subsequently cleared the DNB examination in Plastic Surgery.

Dr. Basu holds fellowship certifications from the St. Andrews Centre for Burns and Plastic Surgery in the United Kingdom, for cosmetic and reconstructive surgery of the breast; from the University of Minnesota in the United States, for facial cosmetic and reconstructive surgery; and from Providence Hospital Medical Centre in the United States, for microvascular surgery.

He has received a number of awards, published extensively in journals and books, and has been an invited speaker at national meetings. He also holds a post-graduate diploma in medical law from the National Law School of India University, Bangalore, and writes on medicolegal questions in surgical practice.

Breast cancer and reconstruction

01What surgery is performed for breast cancer?

Surgical removal remains the mainstay of treatment of a breast tumour. Whether chemotherapy and radiotherapy are also needed is decided case by case.

A patient with a breast tumour may need:

  • Removal of part or all of the affected breast, followed by surgery to restore breast form — either by transferring the patient's own tissue or by using an implant.
  • Removal of lymph glands from the affected armpit. This can cause swelling of the arm on that side, for which surgical correction is possible.
  • Surgical modification of the other breast, at the same operation, to achieve a symmetrical result.
02Can I breastfeed after tumour removal and restorative surgery?

Yes, depending on how much healthy glandular and ductal tissue remains after the tumour is removed. Tissue transplanted from elsewhere in the body provides form, not lactating function — though the breast's job is also to look and feel like a breast, not only to lactate.

03How do I know which surgeries I will need, and what anaesthesia?

You will need to speak to a plastic surgeon experienced in breast reconstruction. Ask your physician or breast surgeon for a referral.

04Why transplant my own tissue rather than use an implant?

Autologous — your own — tissue gives a naturally behaving result and tolerates radiotherapy better than an implanted prosthesis. A flatter abdomen often comes with it. In some situations a prosthesis is the better choice; that is a conversation to have before surgery.

05Will I need further operations after the tumour is removed?

Possibly. Achieving symmetry between the two breasts sometimes requires a further, smaller procedure.

Skin tumours

01What are skin tumours and skin cancers?

Skin cancers most often affect the face and the scalp, and can affect a person of any age or gender. Some kinds appear in early adulthood, others later in life. Prolonged sun exposure, arsenic in drinking water and exposure to human papilloma virus are among the common risk factors in India.

02What are the types?

There is a wide variety, but when detected early most are curable with surgery. It is sometimes possible to biopsy and excise the cancer completely at the same operation.

03Why should a plastic surgeon do the surgery?

The face and scalp matter to a person's appearance, and disfigurement is the primary concern for anyone with a suspicious nodule or mole there.

Oncoplastic and reconstructive surgery have advanced to the point where even large tumours can be removed completely without deforming the face or leaving a bald patch on the scalp. Scars are placed in the natural skin creases so that they are practically imperceptible.

04When should I be concerned?

If you notice a mole or a small ulcer anywhere on your body that is enlarging or not healing, take the opinion of an oncoplastic surgeon. Centres able to test during the operation whether the cancer has been completely excised are essential to proper management.

05Is laser treatment harmful?

If a skin cancer is treated with medicines, radiation or laser without a biopsy, you will never know whether the tumour can come back. Always take the opinion of a certified oncoplastic surgeon before treating a skin cancer.
